In 2003, the band served as an opening act for Dying Fetus and Skinless on a North American tour. From 2006 to 2010, the band's lineup consisted of Jason Netherton on bass and vocals, John Voyles on lead guitar, Mark Kloeppel on rhythm guitar and vocals, and Adam Jarvis on drums. In 2010, Voyles left the group and was replaced by Darrin Morris.

Misery Index performs death metal incorporating elements of hardcore punk and grindcore. The band's lyrics frequently address political, social, and economic themes, including criticism of power structures and aspects of contemporary society. Its name is derived from the economic indicator known as the "misery index" and was also influenced by the final album of the grindcore band Assück, from whom the group has cited inspiration. Between 2002 and 2013, Misery Index performed extensively, playing more than 1,000 concerts across 44 countries.
The band operates its own record label, Anarchos Records, through which it has released several recordings, including the debut EP "Overthrow" (2001), the EP "Dissent" (2004), and the compilation "Pulling Out the Nails" (2010). Misery Index released its debut studio album, "Retaliate", through Nuclear Blast Records in 2003. In January 2006, the band signed with Relapse Records and released its second album, "Discordia", in May of the same year. Its third album, "Traitors", followed in September 2008, and the fourth album, "Heirs to Thievery", was released on May 11, 2010.
In June 2011, Misery Index signed with Season of Mist Records and released its fifth studio album, "The Killing Gods", in May 2014. The band's sixth album, "Rituals of Power", was released on March 8, 2019. In 2020, Misery Index signed with Century Media Records and released its seventh studio album, "Complete Control", in May 2022.
